TERMS AND CONDITIONS:

• Confirmed appointments must be scheduled on Mondays to Saturdays from September 1 to 30, 2025.
• Bundled patients are eligible to receive a free hotel voucher if they avail any of the participating health packages on the same appointment date and time.
• The patients can choose their preferred hotel during the booking period (until supplies last)
• Patients are recommended to schedule their appointment between 07:00 AM to 11:00 AM due to fasting guidelines.
• The patient is responsible for booking their check-in date at the hotel. Cancellations or rescheduling of the check-in date will be managed directly between the patient and the hotel.
• This promo cannot be availed in conjunction with any other existing Shinagawa Diagnostic & Preventive Care promotions and vouchers.
• An Installment mode of payment is not allowed for this promo.
• HMO and Health Insurance members, as well as payment methods through HMO, HI, or company reimbursements, are not eligible for the promo.
• The promotional discount or free items/services are not convertible to cash, discount, or other items. 
• Senior Citizen or PWD discounts cannot be used in conjunction with this promo. However, these discounts may be applied to individual health services if the patient chooses to add riders to their health package.
