How to Book an Appointment
- Patients are required to book through Shinagawa Diagnostic’s Patient Care Line agents and mention the promo code SRC20_2025 (for Category A SRC Members) or SRC10_2025 (for Category B/C SRC Members) to avail of the discount on the health packages.
- Patients may also book directly through the Website Booking Form by selecting “Shinagawa Rewards Circle” in the Membership/Organization dropdown and entering the promo code in the Promo Code field.
- Confirmed appointments must be scheduled between Tuesday, Wednesday, and Thursday, from September 1 to October 30, 2025.
- Patients availing health packages are recommended to schedule their appointment between 7:00 AM to 11:00 AM due to fasting guidelines.
- Rescheduled appointments must be made within the month of September.

Other Terms & Conditions
- Any additional riders or services availed by the patient must be paid in full.
- This promo cannot be availed in conjunction with any other existing Shinagawa Diagnostic & Preventive Care promotions and vouchers.
- Installment mode of payment is not allowed for this promo.
- Senior Citizen or PWD discounts cannot be used in conjunction with this promo. However, these discounts may be applied to individual health services if the patient chooses to add riders to their health package.
- HMO and Health Insurance members, as well as payment methods through HMO, HI, or company reimbursements, are not eligible for the installment payment promo.
- The promotional discount or free items/services are not convertible to cash, discount, or other items.
