How to Book An Appointment:
- Patients are required to book through Shinagawa Diagnostic’s Patient Care Line agents and mention the promo code SRCAT30 or SRCAT10 to avail of the discount on allergy tests.
- Patients may also book directly through the Website Booking Form by selecting “Shinagawa Rewards Circle” in the Membership/Organization dropdown and entering the promo code in the Promo Code field.
- Confirmed appointments must be scheduled only on Monday to Saturday, from August 6 to September 30, 2025.
- Patients are recommended to schedule their appointment between 11:00 AM to 2:00 PM.
- Rescheduled appointments must be made within the month of September.

Other Terms & Conditions
